What If Your Body Has Been Communicating All Along?

Interoception — the perception of internal bodily state — is a continuous communication channel most of us were never taught to read. Learning its vocabulary changes what a person knows about themselves.

Introduction

A great deal of the information that would help us live well is already inside us. It is not held in language. It is held in signal: heart rate, breath depth, gut sensation, muscle tone, temperature, subtle shifts in energy. The channel that carries this signal has a name — interoception — and it has been broadcasting continuously since before we could read. Most of us were simply never taught the vocabulary.

Why This Matters

The person who can read their own interoceptive signals has a source of information the person who cannot read them does not. That information is often what tells us, before any argument arrives, whether a room, a decision, or a relationship is right for us. Learning the vocabulary is one of the highest-yield skills a nervous system can acquire.

The Science

A. D. Craig (2002) mapped the neural substrate of interoception, describing a distinct pathway from body to the insular cortex where internal states are represented as felt experience. Farb and colleagues (2015) reviewed how interoceptive processes contribute to emotion, self-awareness, and psychopathology. Khalsa and colleagues (2018), in a large expert consensus, laid out how deficits in interoceptive accuracy, sensibility, and awareness map onto specific mental-health conditions — anxiety, depression, PTSD, addiction, and eating disorders — and how interventions that improve interoceptive skill improve those conditions. Barrett’s theory of constructed emotion (2017) provides the higher-order picture: the brain uses interoceptive input, together with prior experience, to construct the emotions we then feel. Poor interoception does not just mean not knowing what the body is doing; it means constructing emotions with impoverished data.

Current Research

Recent work has produced tractable interventions. Mindful body-scan practices, breath training, and specific interoceptive exposures (Weng et al., 2021) reliably improve interoceptive accuracy. The improvements transfer: patients with better interoceptive skill regulate emotion more effectively, make better-calibrated decisions under uncertainty, and report higher subjective well-being. The channel can be tuned.

Practical Implications

Learn the vocabulary. Notice what happens in your chest before a hard email lands, in your gut before a decision resolves, in your jaw during a conversation you are not enjoying. Do not assume the sensations mean what they meant last year; keep updating the dictionary. The goal is not to interpret every signal — it is to remain in relationship with a source of information that has been trying to reach you all along.

Common Misconceptions

**“Interoception is just anxiety with a fancy name.”** Anxiety is one construction the brain can build from interoceptive signal. It is not the signal itself. **“The body’s message is always right.”** The signals are real; the constructions we place on them can be miscalibrated. Better interoception improves the calibration — it does not automate the interpretation.
