A person’s sense of purpose is often treated as a philosophical concern. The epidemiology has made it also a medical one.

Why This Matters

If purpose predicts survival, then treating meaning as clinically relevant reorders what belongs in a treatment plan, a workplace design, and a life review.

The Science

Hill and Turiano (2014) followed more than 6,000 adults over 14 years in the MIDUS cohort and found that higher purpose in life predicted lower all-cause mortality, with the effect surviving adjustment for other well-being measures, demographics, and health behaviours. Cohen, Bavishi, and Rozanski (2016) meta-analysed 10 prospective studies covering 136,265 participants and reported that higher purpose in life was associated with a roughly 17% reduction in relative risk of all-cause mortality and cardiovascular events.

Current Research

Contemporary work is disentangling the mechanisms — lower inflammation, better sleep, reduced allostatic load, more consistent health behaviours — and testing whether purpose can be deliberately cultivated in clinical or educational settings.

Practical Implications

For clinicians: asking what a patient is oriented toward is not a soft question. For individuals: the practice of clarifying and reconnecting with meaning is health work.
