Interoception is the ground on which experience is built.

Interoception — the sensing of internal bodily state — was once treated as a marginal topic in neuroscience, one small sense among many. It is now understood to be the substrate on which the rest of experience is built. Emotion, decision, identity, and the very sense of being a self all trace back to interoceptive processing in the insular cortex and its subcortical partners.

The insular cortex as hub

A. D. Craig’s work established the anterior insular cortex as the region where interoceptive signals become subjective feeling — the physiological condition of the body becoming the felt sense of the self.9,10

Neuroimaging has consistently confirmed that interoceptive accuracy correlates with anterior insular gray matter and function.11

Interoception underlies emotion

The theory of constructed emotion (see the Emotion article in this series) treats interoceptive signals as the raw material from which emotions are built. Without body signal, no felt emotion.3,5

Disorders of interoception

Interoceptive dysregulation is now implicated across a wide range of conditions — anxiety, depression, PTSD, eating disorders, chronic pain, functional gastrointestinal disorders, and more. The pattern is remarkably consistent: when interoception is disrupted, so is the experience of self and emotion.12,17,16

The NSI reading

Nervous System Intelligence begins with interoception — with the willingness and skill to feel the body as it is. Not because feeling is a virtue in itself, but because it is the input to everything else.
