Ketamine is a dissociative anesthetic first synthesized in 1962 and approved by the FDA for surgical use in 1970. Over the past two decades, it has emerged as one of the most studied rapid-acting interventions for treatment-resistant depression. Unlike conventional antidepressants that modulate monoamine neurotransmitters over weeks, ketamine acts primarily as an N-methyl-D-aspartate (NMDA) receptor antagonist, producing measurable antidepressant effects within hours to days. Two formulations are now in clinical use: racemic ketamine, administered intravenously off-label, and esketamine, the S-enantiomer delivered intranasally and FDA-approved in 2019 for treatment-resistant depression and depressive symptoms in adults with major depressive disorder and acute suicidal ideation or behavior. The distinction between these formulations matters clinically. Racemic ketamine contains equal parts of two mirror-image molecules (R- and S-ketamine); esketamine isolates the more potent S-form. Evidence suggests they may differ in receptor affinity, metabolite activity, and subjective tolerability, though head-to-head trials remain limited. Ketamine's rapid onset has reshaped the therapeutic landscape for severe depression, but durability of response, risk of dissociation, abuse potential, and the need for repeated dosing complicate its role in long-term care. It offers a pharmacological tool that can interrupt a depressive state rapidly, creating a window in which psychotherapy, behavioral intervention, and nervous system recalibration become more accessible. This is not a cure. Ketamine does not resolve the underlying predictive models that sustain depression, nor does it address the relational, somatic, or environmental factors that contribute to mood dysregulation. But it can create space—a neurobiological pause—in which other forms of work become possible. For clinicians, ketamine introduces both opportunity and complexity. It requires careful patient selection, informed consent regarding dissociative effects and abuse potential, and integration into a broader treatment plan. For patients, it raises questions about durability, cost, accessibility, and the meaning of rapid relief. The stakes are high. Ketamine has been heralded as a breakthrough, but its long-term safety profile, optimal dosing protocols, and role relative to psychotherapy remain areas of active investigation. Understanding what ketamine does—and does not do—matters for anyone navigating severe depression, and for any system of care attempting to support them.

Ketamine's antidepressant mechanism is not fully understood, but converging evidence points to glutamatergic modulation, synaptic plasticity, and downstream signaling cascades involving brain-derived neurotrophic factor (BDNF) and mammalian target of rapamycin (mTOR). A 2023 review in *Nature Medicine* synthesized findings from preclinical and clinical studies, emphasizing that NMDA receptor antagonism alone does not account for ketamine's effects; metabolites, particularly hydroxynorketamine, may contribute independently to antidepressant action (Krystal et al., 2023). This challenges earlier models that attributed efficacy solely to the parent compound. A double-blind, randomized controlled trial published in *The Lancet Psychiatry* in 2022 compared intravenous racemic ketamine to midazolam (an active placebo) in 403 adults with treatment-resistant depression. Depression, from an NSI lens, is not merely a chemical imbalance but a self-reinforcing pattern of prediction: the nervous system anticipates threat, loss, or futility, and organizes perception, affect, and behavior accordingly. These predictions become deeply grooved, resistant to revision through conscious effort alone. Ketamine does not rewrite these predictions. It does not teach the nervous system a new model of the world. What it does is create a temporary suspension—a dissociative pause—in which the usual predictive machinery is offline. This is the Interrupt. In that window, the nervous system is less bound by its habitual forecasts. Affect may lift. Rumination may quiet. Suicidal ideation may recede. But the window is brief. Without deliberate work to Notice what has shifted, Identify the predictions that were driving the depressive state, Regulate the autonomic and affective responses that sustain it, Validate the legitimacy of the nervous system's prior stance, and Align behavior with revised predictions, the old patterns reassert themselves. This is why durability is inconsistent. Ketamine opens a door; it does not walk through it. The NIRVA Method positions ketamine as a catalyst, not a solution. It is most effective when embedded in a broader protocol of nervous system revision—when the pharmacological Interrupt is followed by therapeutic work that helps the system learn something new. This is consistent with emerging clinical models that pair ketamine with psychotherapy, particularly modalities that emphasize interoception, meaning-making, and behavioral activation. Ketamine also implicates the Regulate movement. Dissociation, while often transient, is a form of autonomic dysregulation. The nervous system temporarily loses coherence between interoceptive signals and conscious awareness. For some, this is therapeutic—a relief from the relentless self-monitoring of depression. For others, it is destabilizing. Clinicians must assess whether a patient's nervous system can tolerate this kind of disruption, and whether the context of care is sufficient to support re-integration. The NSI perspective does not diminish ketamine's value. It clarifies its limits. Ketamine is a tool for revision, not a replacement for it. It is most powerful when the person receiving it understands that the relief it offers is an invitation to do the deeper work of updating the predictions that brought them to crisis in the first place.

Ketamine is not appropriate for all patients with depression, and its use requires careful clinical judgment. Candidates for ketamine-assisted treatment typically include adults with moderate to severe major depressive disorder who have not responded to at least two prior antidepressant trials, or those with acute suicidal ideation requiring rapid intervention. Screening should assess for contraindications: uncontrolled hypertension, history of psychosis, active substance use disorder (particularly involving dissociatives or stimulants), and significant cardiovascular disease. Blood pressure monitoring during and after administration is standard, as ketamine transiently elevates blood pressure and heart rate. Informed consent must address dissociation, the potential for psychological distress during infusion, and the lack of long-term safety data beyond two years. Clinicians should also discuss the difference between racemic ketamine and esketamine. Racemic ketamine is administered intravenously, typically at 0.5 mg/kg over 40 minutes, and is used off-label. Esketamine is delivered intranasally at 56 or 84 mg, is FDA-approved, and requires administration in a certified healthcare setting with post-dose observation. Insurance coverage differs; esketamine is more likely to be reimbursed, though cost remains a barrier for many patients. Maintenance protocols vary. Some clinics offer induction series (e.g., six infusions over two to three weeks) followed by as-needed boosters; others use fixed intervals. There is no consensus, and individualization is necessary. Clinicians should monitor not only symptom scores but also functional outcomes, quality of life, and the patient's subjective sense of whether the treatment is helping them move forward. Integration is critical. Ketamine should not be administered in isolation. Concurrent psychotherapy—particularly modalities that support interoceptive awareness, cognitive restructuring, and behavioral activation—enhances durability. Some clinics are exploring ketamine-assisted psychotherapy, in which therapeutic work occurs during or immediately after the dissociative state, though evidence for this approach is still emerging. Clinicians must also be alert to the risk of dependence. While rates of problematic use in supervised settings are low, ketamine has recreational appeal, and patients with histories of substance misuse require additional safeguards. Finally, ketamine is not a substitute for addressing social determinants of health, trauma, or relational rupture. It is one tool among many, and its effectiveness is amplified when embedded in comprehensive, person-centered care.

If you are considering ketamine for depression, begin by asking whether you have exhausted other evidence-based options. Have you tried at least two different classes of antidepressants at adequate doses for adequate durations? Have you engaged in psychotherapy, particularly cognitive-behavioral or interpersonal approaches? Have you addressed sleep, nutrition, movement, and social connection? Ketamine is not a first-line treatment. It is a tool for when other tools have not worked, or when the urgency of your situation requires rapid intervention. If you proceed, choose a provider carefully. Look for a clinic that conducts thorough psychiatric evaluation, monitors vital signs, provides a calm and supportive environment, and offers integration support. Ask whether they use racemic ketamine or esketamine, and why. Ask about their maintenance protocol and how they will support you between sessions. Understand that the experience may be disorienting. Dissociation can feel like floating, detachment, or a dreamlike state. For some, this is peaceful; for others, unsettling. You will be monitored, and the effects are temporary. What matters more is what happens after. In the hours and days following treatment, pay attention to shifts in mood, energy, and thought patterns. This is the window. Use it. If rumination has lifted, notice what it feels like to think without that weight. If suicidal ideation has receded, notice the space that opens. This is not the time to assume you are fixed. It is the time to begin revising the predictions that brought you to this point. Work with a therapist to identify the beliefs, expectations, and autonomic patterns that sustain your depression. Practice interoceptive awareness: what does your body feel like when the depression is lifting? What does it feel like when it returns? Use that information to Regulate—through breath, movement, connection, or rest. Do not expect ketamine to do the work for you. It will not. But it can give you a foothold. It can show you that change is possible, that your nervous system is not locked in place. What you do with that knowledge determines whether the relief lasts.
