Definition

Placebo and nocebo effects, understood through predictive processing, are the neural consequences of expectation. Meaning changes prediction; prediction changes experience; experience feeds back into biology.

Why it matters

This framework preserves both the reality of placebo effects and the reality of the underlying suffering. It also clarifies how clinician communication becomes pharmacology.

The Science

Placebo analgesia recruits endogenous opioid, dopaminergic, and cannabinoid systems. Nocebo effects can produce real symptoms. Both are shaped by conditioning, expectation, and social context.
