Definition
Persistent pain after healing is pain that continues after tissues have measurably recovered. It is not imaginary; it is often a case of a nervous system that has learned pain and continues to produce it in the absence of ongoing damage.
Why it matters
This is one of the most misunderstood pain phenomena. Patients are often told there is “nothing wrong” and left without help. The neuroscience says otherwise — and offers direction.
The Science
After injury, plasticity in spinal cord and cortex can outlast tissue healing. Cortical maps can distort. Central sensitization can persist. Phantom limb pain is the extreme case: the tissue is gone, and pain remains. Mirror therapy, graded motor imagery, and pain neuroscience education can reduce pain by restoring accurate sensorimotor input and updating predictions.
The NSI Perspective
NSI names this experience honestly: the nervous system has learned pain, and the learning outlasted its usefulness. This framing is both accurate and hopeful.
Clinical Implications
Care includes clear education, sensorimotor retraining, graded exposure, and, when appropriate, mirror-based techniques. Repeat surgeries and escalating opioids are usually not the answer.
Practical Application
If tissues have healed but pain remains, you are not making it up. Your nervous system is doing what nervous systems can do. It can, with the right help, also unlearn.
