Definition

Pain predictions are one of the clearest applications of predictive processing to clinical experience. The brain constructs pain in part from its forecast that the body needs protection, and updates that forecast with evidence.

Why it matters

This framework explains persistent pain, placebo, nocebo, and phantom limb phenomena in a coherent way — and it points directly at how care can help.

The Science

Studies of pain expectation, placebo analgesia, and mirror therapy show that pain intensity is shaped by predicted threat and can be reduced by prediction-updating experiences. This does not make pain imaginary; it clarifies its construction.

NSI holds pain-as-prediction as one of the most important applications of predictive processing to clinical care.

Clinical Implications

Interventions that update pain predictions — accurate education, safe movement, mirror therapy, gradual exposure — are aligned with the underlying biology.

Practical Application

Every safe, unexpected experience that contradicts a pain prediction is a small piece of medicine.
