Definition

Many medications — including antibiotics, PPIs, opioids, SSRIs, metformin, NSAIDs, and chemotherapy — substantially alter gut function, microbiome, or both.

Why it matters

Gut-related side effects are among the most common medication complaints, and they can drive discontinuation. Naming them accurately supports adherence and better decisions.

The Science

Antibiotics reshape microbiome composition, sometimes durably. PPIs alter gastric pH and downstream microbiome. Opioids slow motility. SSRIs commonly produce transient GI symptoms via serotonergic effects. Metformin alters microbiome and can produce reversible GI symptoms.

NSI holds medication side effects as legitimate information, not personal weakness. Shared decision-making requires honesty about tradeoffs.

Clinical Implications

Reviewing medications when gut symptoms arise is standard careful practice.

Practical Application

If a medication changed your gut, that is real. Talk to your prescriber. Do not just push through in silence.
