Definition
Central sensitization is a state in which the central nervous system amplifies pain signals, so that non-painful stimuli feel painful (allodynia) and painful stimuli feel more painful (hyperalgesia).
Why it matters
Central sensitization explains why many people with chronic pain have pain that is disproportionate to visible tissue damage, and why standard tissue-focused treatments often fail. It is a real, measurable neurological state.
The Science
Central sensitization involves increased synaptic efficacy in dorsal horn neurons, loss of descending inhibition, altered glial activity, and reorganization of cortical maps. It has been documented in fibromyalgia, chronic low back pain, complex regional pain syndrome, and post-amputation phantom limb pain. Mirror therapy, first popularized by Ramachandran, appears to work partly by restoring accurate sensorimotor input and reducing sensitization-linked cortical distortion.
The NSI Perspective
NSI treats central sensitization as a learned protective state of the nervous system — not a character flaw, and not proof that the pain is imaginary. The system has learned an amplified language of threat and can, with skilled help, learn a quieter one.
Clinical Implications
Treatment includes pain neuroscience education, graded motor imagery, mirror therapy, gentle graded exposure to feared movement, sleep restoration, and stress regulation. Escalating tissue-focused interventions typically do not resolve central sensitization.
Practical Application
If light touch, cold, or gentle movement produces disproportionate pain, this is neurology behaving predictably. It is not you exaggerating. It is a nervous system that has learned to protect too vigorously — and it can be helped.
