Can Someone Feel Safer Without Losing Awareness of Risk?

Definition

A goal of MDMA-assisted therapy is to reduce the overwhelming fear that blocks trauma processing while preserving the patient’s judgment, agency, and awareness of context.

Why it matters

Reducing fear too much can compromise the very cognitive functions needed for integration. Preserving awareness protects consent and long-term outcomes.

The Science

Trial protocols emphasize titrated dosing and monitoring so that patients remain oriented and able to communicate. Loss of awareness is a clinical risk, not a therapeutic aim.

The NSI Perspective

Safety is not sedation. NSI treats treatment as a collaborative encounter, not a bypass.

Clinical Implications

Screening should exclude patients likely to become severely dissociated. Monitoring during sessions is essential.

Practical Application

If exploring this care, ask how the clinician handles moments of overwhelm, and how they preserve your capacity to consent throughout the session.
