After Polyvagal Theory: What the 2026 Debate Means for Nervous System Science

The scientific community is actively re-evaluating one of the most influential frameworks in nervous system science. Here’s what the debate means — and why it matters for anyone building their understanding of how their nervous system works.

The 2026 Grossman–Porges Exchange

In early 2026, Clinical Neuropsychiatry published what may become one of the most significant exchanges in autonomic neuroscience this decade. Paul Grossman and colleagues released a comprehensive evaluation arguing that Polyvagal Theory (PVT) — Stephen Porges’s influential framework connecting vagal function to social behavior and safety — is untenable based on current neurophysiological and evolutionary evidence.

In the same issue, Porges published a scholarly response arguing that the critique attacks a misconstructed version of the theory rather than PVT as it is actually formulated in the peer-reviewed literature.

The debate centers on three core issues:

Anatomical Claims

Whether PVT’s separation of ventral and dorsal vagal pathways into distinct functional systems is biologically defensible given current neuroanatomical evidence.

Measurement Validity

Whether respiratory sinus arrhythmia (RSA) serves as a valid proxy for vagal tone across different physiological conditions — a measurement assumption underlying much PVT-based research.

Evolutionary Framework

Whether PVT’s phylogenetic hierarchy — the idea that mammals evolved a social engagement system layered on top of older defensive circuits — is supported by comparative evolutionary data.

What both sides agree on is significant: this is a test of the theory’s core mechanisms, not merely its clinical popularity. What they disagree on is whether Grossman et al. evaluated PVT on its own terms or refuted a simplified version of it.

Evidence Hierarchy: Where Does Polyvagal Theory Stand?

Applying a rigorous evidence hierarchy — the kind of intellectual honesty that nervous system science deserves — Polyvagal Theory currently sits at the level of Supported Finding: clinically useful, widely applied in therapeutic contexts, but not yet fully validated at the mechanistic level across the claims it makes.

This is not a dismissal. Supported Findings are valuable. Clinicians worldwide report that PVT-informed approaches help patients understand their responses to threat. Trauma therapists use its language to normalize nervous system reactions. These clinical observations are real and meaningful.

But clinical utility and mechanistic validity are different questions. A model can be therapeutically helpful (because it gives people language for their experience) without every biological claim being confirmed. The 2026 debate forces the field to hold both truths simultaneously.

Biological Humility: A Necessary Posture

The 2026 debate reveals something the field has needed to acknowledge for years: no single theory owns the nervous system.

We propose a posture we call Biological Humility — the practice of using helpful models while remaining genuinely open to new data. This isn’t intellectual weakness; it’s scientific maturity. It means:

Biological Humility is the opposite of dogmatism. It doesn’t require practitioners to abandon PVT-informed approaches that help their clients. It asks them to hold those approaches with appropriate confidence — neither dismissing them as disproven nor defending them as settled science.

Why Nervous System Intelligence Doesn’t Depend on Any Single Theory

Nervous System Intelligence (NSI) was designed for exactly this moment — not because we predicted the Grossman–Porges exchange, but because the framework was built on a different foundation entirely.

NSI is not a theory about vagal pathways. It’s not a model of evolutionary neurology. It is a framework for understanding your own automatic nervous system patterns: what shaped them, how they operate, and what becomes possible when you develop genuine understanding of your own responses.

This is not a criticism of PVT. It’s an architectural choice: build on understanding rather than on any single theory, and the framework remains useful as science evolves.

What This Means for Practitioners

If you’re a therapist, coach, nurse, or educator who uses PVT-informed language with clients:

You don’t need to stop.

The clinical observations that make PVT language useful — that people have different nervous system states, that safety influences regulation, that social connection affects physiology — are supported at levels above PVT’s specific mechanistic claims.

You do need to be honest.

When you say your ventral vagal system is coming online, you’re using a helpful metaphor. Call it that. Don’t present it as established neuroscience when the underlying mechanism is actively debated.

Consider adding a discovery layer.

If you’ve encountered Polyvagal Theory through social media, books, or a therapist:

The concepts that helped you are not suddenly wrong. If understanding fight, flight, freeze, or fawn gave you language for your experience, that language is still useful. If learning about your nervous system helped you feel less broken, that understanding still matters.

What the 2026 debate means is that the specific biological story behind those concepts is more complex than any single theory captures. Your experience is real. The science explaining it is still evolving. Both things are true.

The Grossman–Porges exchange is not an ending. It’s a maturation point for the field of nervous system science. The questions being asked — about mechanism, measurement, and evolution — will produce better science, more honest education, and ultimately more effective approaches to understanding human nervous system function.
